What that requires in practice

Organisational signals · Governance & systems · Learning & adaptation · Measurement & insight · Leadership & accountability

All five are examined in every organisation. They are rarely in balance and that imbalance is often where organisational capability begins to break down. A strong system with no accountability is not a safe system. Where does your organisation stand on all five?

Read them and count your own answers.

1. Organisational signals: Can you name one psychosocial risk you assessed in the last twelve months and say where the signal came from?

2. Leadership & accountability: Can you name the person accountable for the control that followed? A name, not a function.

3. Governance & systems: Does that person have the mandate to change work design, staffing or schedules, without escalation?

4. Measurement & insight: Can you show what changed because of it, in work design rather than in a survey score?

5. Learning & adaptation: Has any concern raised in the last two years changed how work is designed, allocated or led?

If two or more of those are "no" or "not sure", you have the structure but not the evidence. That is the gap an inspection finds and it is structural, not a matter of effort.
